Insurance Policy
Dr. Krafchick is an in-network provider with Tricare Insurance only.
For all other insurance plans, Dr. Krafchick is an out of network provider and will not bill your insurance for payment. Payment is due at the time of service. Payments may be made via check, cash, credit card, or from a health savings account/card. We will provide a "Superbill" with all the information you need to submit to your insurance company.
Dr. Krafchick is not a Medicaid provider (Health First Colorado).
Fee Schedule
$385 per 90-minute new patient assessment (patients 18 and younger)
$280 per 60-minute new patient assessment (patients over 18 or if patient hasn’t been seen in 2+ years)
$225 per 50-minute follow up combined therapy/medication management appointment
$140 per 25-minute follow-up medication management
